VBAC, Vaginal Birth after Cesarean and Acupuncture

Acupuncture for labor preparation is ideal for women who had previous cesarean births who are planning a natural birth.

Acupuncture will prepare and ripen the cervix naturally, acupuncture will also support the baby into an optimal position.



Facts

In its 1995 bulletin, the American Congress of Obstetricians and Gynecologists recommended that repeat cesareans be limited, and encouraged a trial of labor with vaginal birth even after two previous cesareans. Nevertheless a vaginal birth after c-section is still a rarity in North America with 90% repeat cesarean rates.

It is also reassuring to report that on average 60-90% of women who attempt a Vaginal Birth After Cesarean succeed.


Is a VBAC the Right decision?

Of course, this is a discussion for you to have with your healthcare practitioner. But generally speaking, if you have a low-risk pregnancy, you are a great candidate for a natural VBAC birth.

Some considerations when thinking about a VBAC include the following:

· Type of surgical incision from the previous cesarean

· Reason for the past cesarean

· How many previous cesareans one has had

· The mother’s health issues, if any

· The shape of the uterus

· The presentation of baby



Benefits of vaginal birth after c-section


The hormones produced during labor have major benefits (even if it ends in another cesarean!).

  • Oxytocin ripens the cervix and has a calming, analgesic effect on both mother and baby.

  • Oxytocin assists with the bonding between baby and mother.

  • Oxytocin helps to shrink the uterus and decrease the chances of postpartum hemorrhage.

  • Rising estrogen levels prepare the uterus for labor, promote breastfeeding and also reduce the likelihood of hemorrhaging.

  • Promote baby's breathing; and boost their immunities.

  • Recent research shows that it prevent baby from developing allergies and asthma.

Does research support pre-birth acupuncture?


Kimberley Laurence citing Dr Debra Betts (PhD) reports that an observational study compared the birth outcomes of women who had received pre-birth acupuncture with those who received usual care (https://www.kimberleylaurence.com.au/preparing-for-a-natural-birth-with-acupuncture)


The study identified a number of positive outcomes of acupuncture treatment including:



  • The use of acupuncture in pregnancy is safe

  • A 35% decrease in the number of inductions (for women having their first baby this was 43%)

  • A 31% reduction in the epidural rate.

  • A 32% reduction in emergency cesarean delivery

  • There was no statistical difference in the onset of early labor in those women receiving pre-birth acupuncture.



When should I start?


For people who have had a cesarean, it is ideal to have acupuncture treatment following surgery and before the conception of a second child. This helps to heal scar tissue and increase the possibility of VBAC (vaginal birth after cesarean.)

Acupuncture after any surgery helps with pain and quickens recovery all together, but is particularly useful if you are wanting to avoid a second C-section.

During pregnancy, treatments start around 36 weeks of pregnancy, however pre-birth acupuncture can still be beneficial if started after this date.
